What is EX-17?

The EX-17 is the application form for Spain’s Tarjeta de Identidad de Extranjero (TIE, Foreigner Identity Card).

EX-17 field by field

Use version 03 of the form. Write in block capitals with a black ballpoint pen, or type it. Use dates in dd/mm/yyyy format.

Download and submit EX-17

Get the official EX-17 form from the Ministry. The form itself is free. The separate fee is Modelo 790 código 012, paid before your fingerprint appointment.

Submission is only in person at the competent National Police unit. Online submission and public-register submission aren't accepted. Check the official National Police information for the appointment and office details.

Steps to complete it

1. Download the form. Use the official EX-17 PDF.
2. Fill in your details. Complete sections 1 and 3. Complete section 2 only if a representative submits for you.
3. Choose the application type. Tick one box in section 4 only.
4. Print and sign. Print the form twice. Sign both copies.
5. Attend your appointment. Bring both copies to your fingerprint appointment with the Modelo 790 code 012 fee receipt.

Documents to take to your appointment

Prepare these papers before you go to the National Police unit.

• The completed EX-17 original and one copy, both signed.
• The receipt showing payment of Modelo 790 code 012.

Common problems and how to fix them

Wrong application type

Print a fresh form and tick only the correct box: initial card, renewal, or duplicate. The EX-17 form is free.

Names do not match

Copy your surnames and given name exactly from your passport. Replace the form if you already wrote a different version.

Empty notification address

Enter the address where official letters can reach you. Don't leave section 3 blank.

One copy signed

Sign both printed copies. Bring the original and the copy to your appointment.
